Who we are

Rejuve-Nation Wellness Clinic operates a chiropractic and wellness clinic at 797 Princess Street, Kingston, ON. You can reach us at (613) 542-5802 or docgiven.getwellstaywell@gmail.com.

Information we collect

  • Website inquiries. When you submit our contact form or email us, we receive your name, email, phone (if provided), and message.
  • Patient records. As a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA), we collect and hold health history, assessment findings, imaging, and treatment notes needed to provide care.
  • Basic site analytics. Our hosting provider may log standard technical information (approximate location, browser type, pages viewed) to keep the site secure and running well. We do not use advertising trackers.

How we use your information

  • Respond to inquiries and appointment requests.
  • Provide chiropractic assessment, treatment, and follow-up care.
  • Prepare receipts for insurance reimbursement (at your direction).
  • Meet our professional, regulatory, and legal obligations.

Sharing your information

We do not sell your information. We only share personal health information with your consent, or where required by law — for example, with a referring practitioner you have asked us to communicate with, or with the College of Chiropractors of Ontario when required.

Your rights

You may request access to your personal health record, ask us to correct an inaccuracy, or withdraw consent to future collection where the law permits. Contact the office to make a request or to raise a concern about our privacy practices.

Retention and security

Patient records are retained for the period required by the College of Chiropractors of Ontario. Paper records are stored securely at the clinic; electronic records are stored on access-controlled systems.
